are an important part of
Hope School!

A volunteer form must be completed and submitted to the Office for approval. Tuberculosis test results must also be submitted
and kept on file. TB test must be updated every 4 years.

If you are planning to help with driving on field trips for your child's class, the District requries that you carry the following insurance coverage:

$100,000/300,000 Liability
$50,000 Property Damage
$2,000 Medical Payment

You will need to fill out a "Field Trip by Private Vehicle" form and provide a copy of the face sheet of your policy to Judy in the office for approval.
